By signing this Volunteer Form, I acknowledge and agree that I may be photographed, recorded, or filmed while participating in the Gratitude event, volunteer activities, events, programs, and other church-related activities.
I grant the church and its authorized representatives permission to use my name, image, likeness, voice, photographs, video recordings, and other media in which I appear for church event-related purposes, including but not limited to church websites, social media, livestreams, newsletters, promotional materials, publications, advertisements, and other media outlets.
I understand that these materials may be publicly viewed and may be edited, reproduced, distributed, or displayed without additional notice or compensation. I waive any right to inspect or approve the final materials before they are used.
By joining the volunteers team, I confirm that I have read and understood this Media, Photo & Video Release and voluntarily provide my consent.
